Intake and Assessment

The intake and assessment services that St. Mary’s Home Health provides are remarkable in themselves.

Sometimes people just wonder, “What can I get for Mom?” or,  “Would we qualify for anything?” Our intake person at St. Mary’s Home Health hears these questions all the time. and she can be a great first step toward getting your questions answered. After that, our assessment team can help you learn more about exactly what you or loved one needs and what we can do to help. 

After you call us, our intake and assessment process begins:

  1. Insurance verification.  We will do the legwork for you to determine what kind of services your insurance company will cover. 
  2. Initial assessment. Based on the physician's orders and your needs, the applicable skilled professional will come to your home for an initial assessment. If you need service right away, this visit can happen within 24 hours of intake. The professional will gather vital information about your health and talk with you and your loved ones about safety and many other concerns.
  3. Therapy assessments. If it is warranted, a Therapist will come to your home for an initial visit, or a Nurse can call in the expertise of other members of our professional team. For instance:
  • A Physical Therapist may do movement, safety, mobility and/or equipment assessments;
  • An Occupational Therapist may consider a wide range of functional issues, like cognition, memory, vision, or safety in the kitchen;
  • Our Speech Therapist may assess oral motor skills like swallowing, breathing and chewing, as well as your communication skills;
  • Our Social Worker may assess your support systems and eligibility for other services.
