Our caring social worker has more than a decade of experience in the medical world — counseling families as they go through difficult transitions and helping them find needed resources in the community. She will offer options that can enhance your quality of life.

The social worker can facilitate services as basic (yet invaluable) as getting a grocery service or cooking service in place, or helping a client feel empowered to call the neighbors to ask them to check on him or her every day. Alternatively, the social worker could help you get set up with a more formal lifeline service. She can also provide:

• Referral to extended home health care services, or alternative housing situations like assisted living or long-term care facilities;
• Counseling regarding financial stress and guardianship issues; and
• Referral and advocacy for financial assistance.
